Annual Membership Fee

If you pay for the U.S. Dental Care Access Card in full at the time of purchase, the following fees apply:

Enrolling one person: $89

Enrolling two people: $139

Enrolling family (spouse and dependent children): $169

Monthly Payment Schedule

If you pay for the U.S. Dental Care Access Card in monthly installments, the following fees apply:

Enrolling one person: $8 per month

Enrolling two people: $12 per month

Enrolling family (spouse and dependent children): $15 per month

The initial payment must include the first two months’ membership fees. (Example, the initial payment for one person enrolling is $16 to cover the first two months. After that, a monthly fee of $8 per month applies.)

Cancellation Policy

If you cancel within 30 days of the date of purchase, the membership fee(s) you paid are refundable.
